1. Agreement to These Terms
This website (the "Site") is owned and operated by ReShapesRx & Wellness ("ReShapesRx," "we," or "us"), located at 3211 Cummings Highway, Chattanooga, TN 37419. By accessing the Site, submitting a form, booking an appointment, or receiving services from us, you agree to these Terms and Conditions and to our Privacy Policy.

Accessing the Site in any manner constitutes use of the Site and acceptance of these terms. We may revise these terms at any time by posting an updated version here. Continuing to use the Site after we post changes means you accept the revised terms.

2. Medical Disclaimer
The content on this Site is provided for general informational purposes only. It is not medical advice, and it does not create a provider-patient relationship. Nothing on this Site should be used to diagnose or treat a health condition or to replace a conversation with a qualified medical provider.
All treatments require an in-person consultation and evaluation by one of our licensed providers
Not every patient is a candidate for every service, and we reserve the right to decline treatment based on clinical judgment
Individual results vary. We do not guarantee any particular outcome, duration of effect, or result timeline
Before-and-after images shown on our site or social channels reflect the results of specific individuals and are not a promise of what you will experience
All treatments carry risk. Risks, benefits, alternatives, and aftercare requirements are reviewed with you and documented in your written informed consent before any treatment is performed

You are responsible for providing complete and accurate information about your medical history, medications, allergies, and prior treatments
3. Appointments, Deposits, and Cancellations
A deposit or card on file may be required to reserve certain appointments. Deposits are applied to the cost of your service
We ask for at least 24 hours’ notice to cancel or reschedule
Cancellations inside that window, no-shows, and same-day reschedules may result in forfeiture of your deposit or a fee of $50
Arriving more than 15 minutes late may require rescheduling, and the missed-appointment policy may apply
Consultation fees, where charged, may be credited toward treatment received within 30 days

4. Pricing, Payment, Packages, and Gift Certificates
Pricing is subject to change without notice. Quoted pricing is valid for the period stated at the time it is given
Payment is due at the time of service unless other arrangements have been made in writing
Because aesthetic and wellness services are performed on the body and cannot be returned, all services rendered are final and non-refundable
Packages, series, and prepaid treatments are non-refundable and non-transferable, and expire 12 months from the date of purchase
Memberships are governed by the separate membership agreement provided at enrollment, including its billing, pause, and cancellation terms
Gift certificates are non-refundable, may not be redeemed for cash, and are subject to applicable Tennessee law
Retail skincare may be returned only if unopened and unused, within 14 days of purchase
Touch-ups, adjustments, and follow-up assessments are handled at our clinical discretion and according to the policy for the specific service

5. Promotions, Offers, and Model Opportunities
Promotions, specials, referral rewards, and model call opportunities are offered at our discretion and subject to their own stated terms.
Offers cannot be combined with other offers, discounts, or manufacturer rewards unless we state otherwise
Offers have stated start and end dates and may be limited by availability or by number of participants
Manufacturer rewards and loyalty programs are administered by the manufacturer under its own rules, and eligibility is determined by that manufacturer, not by us
Model opportunities require completion of our application and screening process, and selection is at our discretion. Applying does not guarantee selection, and participation typically requires consent to photography and to the presence of observers or trainees

6. SMS Messaging Terms
Program description. ReShapesRx & Wellness operates text message programs that send appointment confirmations, appointment reminders, care instructions, scheduling updates, and replies to your questions, and — separately, and only with your additional consent — offers, events, and other promotional messages. Consent is collected through a dedicated checkbox on our web forms, through written request, or in person at the practice. Message frequency varies.
Consent is never required as a condition of purchase or of receiving care. You must be 18 or older to participate.
Cancellation. Text STOP to any message from us at any time. We will send one confirmation message and stop sending. To resume, opt in again through any of our forms or contact the practice.
Support. Reply HELP to any message, email reshapesrxandwellness@gmail.com, or call (423) 250-3339 during business hours.
Carriers and rates. Message and data rates may apply to messages sent to and from us. Carriers are not liable for delayed or undelivered messages. The program is supported by all major U.S. carriers, including AT&T, Verizon, T-Mobile, and most regional carriers. Contact your wireless provider with questions about your plan.

7. Intellectual Property
The Site and its contents — including text, graphics, logos, photographs, video, and design — are the property of ReShapesRx & Wellness or its licensors and are protected by copyright, trademark, and other intellectual property laws. The Site is provided for your personal, non-commercial use.
You may not modify, copy, reproduce, republish, upload, post, transmit, sell, create derivative works from, or otherwise distribute any material from the Site without our written permission. You may download or print individual pages for personal, non-commercial use so long as all proprietary notices remain intact.

8. Content You Submit to Us
When you submit content to us — including reviews, comments, photographs, video, and social media posts that tag or mention us — you confirm that you own that content or have permission to share it, and that it does not violate anyone else’s rights. You grant us a royalty-free, non-exclusive, worldwide license to use, reproduce, adapt, publish, and display that content in connection with our marketing and operations. Clinical before-and-after photography is governed separately by the written photo authorization you sign at the practice, not by this section.
You agree not to submit content that is unlawful, defamatory, misleading, or infringing, and we may remove any submission at our discretion.

9. Registration and Account Security
Some features may require you to create an account or patient portal login. You agree to provide accurate and current information and to keep your credentials confidential. You are responsible for activity that occurs under your account. Notify us immediately at reshapesrxandwellness@gmail.com if you suspect unauthorized use.

10. Third-Party Links and Vendors
The Site may link to websites and services operated by others, including scheduling, payment, financing, and manufacturer rewards platforms. We provide those links for convenience and do not control or endorse those sites. Your dealings with a third party are between you and that third party, and we are not responsible for their products, services, availability, accuracy, or handling of your information.

11. Disclaimer of Warranties
The Site and the information on it are provided "as is" and "as available," without warranties of any kind, express or implied. To the fullest extent permitted by law, we disclaim all warranties, including implied warranties of merchantability, fitness for a particular purpose, and non-infringement. We do not warrant that the Site will be uninterrupted, error-free, or free of harmful components.

12. Limitation of Liability
To the fullest extent permitted by Tennessee law, ReShapesRx & Wellness and its owners, providers, employees, and agents will not be liable for indirect, incidental, special, consequential, or punitive damages arising out of your use of the Site or reliance on its content. Nothing in these Terms limits any liability that cannot be limited under applicable law, and nothing here alters the standard of care applicable to medical treatment you receive at our practice.

13. Indemnification
You agree to indemnify and hold harmless ReShapesRx & Wellness, its owners, employees, and agents from any claims, damages, liabilities, costs, and expenses, including reasonable attorneys’ fees, arising from your breach of these Terms, your misuse of the Site, or content you submit.

14. Termination
We may suspend or terminate your access to the Site at any time, without notice, if we believe you have violated these Terms or engaged in conduct that is unlawful or harmful. Upon termination, you must stop using the Site and any content obtained from it.

15. Governing Law and Dispute Resolution
These Terms are governed by the laws of the State of Tennessee, without regard to its conflict of law rules. Any dispute arising out of or relating to these Terms or your use of the Site will be brought exclusively in the state or federal courts located in Hamilton County, Tennessee, and you consent to the jurisdiction of those courts.

We may update these Terms from time to time. The current version will always be posted here with its effective date.
